By accessing or using the PRISM Medical Care website and services, you agree to the following terms and conditions. If you do not agree, you should not use this website or services.

Use of Services

PRISM Medical Care provides telehealth services by a licensed healthcare provider. Services are intended for non-emergency medical conditions and are subject to clinical appropriateness.

Use of this website does not establish a provider-patient relationship unless and until a formal visit is completed.

No Emergency Services

PRISM Medical Care does not provide emergency care.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

Eligibility

Services are provided only to patients located in states where the provider is licensed at the time of the visit. You may be required to verify your identity and location.

Medical Services Disclaimer

All medical decisions are based on information provided during the visit. Incomplete or inaccurate information may affect care.

PRISM reserves the right to decline treatment if a condition is not appropriate for telehealth.

Payments & Billing

Payment is due at the time of service unless otherwise billed through insurance. Patients are responsible for understanding their insurance coverage.

Cancellations & No-Shows

Cancellation & No-Show Policy

At PRISM Medical Care, we value your time and strive to provide timely, efficient care to all patients.

We kindly ask that you cancel or reschedule your appointment at least 24 hours prior to your scheduled appointment time.

Late Cancellations & No-Shows

Appointments canceled or changed less than 24 hours in advance will be subject to a $30 late cancellation fee

Failure to attend a scheduled appointment without notice (no-show) will also result in a $30 fee

Payment Responsibility

By scheduling an appointment, you acknowledge and agree to this policy and authorize PRISM Medical Care to charge the applicable fee to your card on file.

Exceptions

Fees may be waived at the sole discretion of PRISM Medical Care in cases of emergencies or extenuating circumstances.
