For purposes of these policies, “PRISM Care,” “we,” “our,” and “provider” refer to PRISM Care, a DBA of PRISM Care PLLC, including its providers, employees, contractors, and authorized representatives.

Patients must provide accurate information and follow treatment plans.

2. Follow-Up

Required for labs and chronic conditions (typically every 3 months).

3. Non-Compliance

May result in increased risk or discontinuation of care.

4. Liability

PRISM Care not liable for outcomes due to non-compliance except gross negligence
