For purposes of these policies, “PRISM Care,” “we,” “our,” and “provider” refer to PRISM Care, a DBA of PRISM Care PLLC, including its providers, employees, contractors, and authorized representatives.
1. Purpose
This policy defines the scope of services and clinical limitations of telehealth in accordance with federal law, Medicare standards, and Nevada regulations.
2. Scope of Services
PRISM Care provides telehealth-based acute care (non-emergent), chronic disease management (stable conditions), preventive care, limited mental health care, and care coordination.
3. Pediatric Limitation
Services for ages 3+ are limited to simple acute pediatric conditions. Preventive and chronic pediatric care are excluded.
4. Clinical Limitations
Telehealth lacks physical examination and relies on patient-reported information, which may limit diagnostic accuracy.
5. Excluded Conditions
Conditions requiring emergency or in-person care (e.g., chest pain, shortness of breath, neurological deficits, severe pain, bleeding) are excluded.
6. Referral & Escalation
PRISM Care may require in-person evaluation or refer to specialists or emergency services when clinically indicated.
7. Standard of Care
All services meet the same standard of care as in-person services per Medicare and Nevada law.
8. Limitation of Liability
PRISM Care is not liable for outcomes related to telehealth limitations or patient-provided information, except in cases of gross negligence or willful misconduct.
