Notice of Privacy Practices (NPP)

For purposes of these policies, “PRISM Care,” “we,” “our,” and “provider” refer to PRISM Care, a DBA of PRISM Care PLLC, including its providers, employees, contractors, and authorized representatives.

All disputes resolved through binding arbitration.

2. Waiver

Waive right to jury trial.

3. Scope

Applies to PRISM Care and affiliates.

4. Process

Conducted under applicable arbitration rules in Nevada.

5. Severability

Invalid provisions do not affect the remainder.
